There are still two Tad's Steaks locations in Manhattan:
152 West 34th Street, Manhattan, NY - (212) 630-0318 (near Penn Station)
1515 Broadway, New York, NY - (212) 626-7305 (theater district)

I did eat at the Penn Station location many years ago and decided I could do better elsewhere; steak never was my thing but I could what was good and what wasn't.

They now charge something like $12.99 for dinner, $8.99 for lunch, and up.  They don't have their own web page.  Read the reviews before going there, and YOU HAVE BEEN WARNED.
