Normally the RR would engage the local communites, the County or the State 
(depending upon whether it was a local, County or State route) to repair a 
span...

Nowadays, when a RR-owned structrure needs to be replaced, the RR will go to 
the appropriate governmental agency to make a deal for the RR to rebuild the 
new bridge if the local agency will take over maintenance responibility, 
since the RR's are not in the business of maintaining road bridges.

As an example, here in Ohio, CSX has two (2) bridges east of Ravenna over 
the old-B&O mainline that CSX is clearing for double-stack traffic (aka 
CSX's National Gateway iniative).  These two spans are old iron/wood 
truss/wood plank bridges in terrible condition.  CSX plans to remove and 
rebuild those two spans at their cost, with Portage County agreeing to take 
over maintanence responsiblity upon completion.  I'm sure this kind of 
arrangement has happened on other portions of the line, and does happen here 
in Ohio...
